MEGA Swivel 10K Steel Anchor + Weld-On Puck Combo

This combo kit packages Frontline's two-piece steel anchorage system in one box: the STM10K-SA MEGA Swivel 10K anchor and the STMP10K-WO weld-on puck it mounts to. The anchor's revolutionary swivel ring design lets the D-ring rotate 360 degrees and flip 180 degrees side to side, with heavy gauge heat treated alloy steel, a reinforced bridge, and a patent pending key lock that keeps the D-ring seated and locked in its swivel housing under heavy loads.

The 3 x 1 inch unplated mild steel puck welds permanently to structural steel with a 3/8 inch minimum fillet, and the anchor threads on with the included plated Grade 8 5/8-11 bolt and internal tooth lock washer - rated up to 10,000 lbs tensile strength, supporting permanent and temporary installations on steel. The welding procedure must be approved by an engineer or other qualified person for the installation location.

Who Needs This Product

This product is a complete two-piece steel anchorage kit - the permanent weld-on puck plus the removable 10K swivel anchor that rides on it. Per our kit rule it lives alongside its components: buy the combo when you are starting a new anchorage location, add bare pucks to extend the route, and carry one swivel anchor between them. Each worker still needs a full body harness and a compatible connecting device.

Welding is engineering-controlled. The welding procedure must be approved by an engineer or other qualified person as compatible with the structural and operational characteristics of the installation location, and the weld must meet the 3/8 inch minimum on the installation drawing. One worker per anchor - verify the completed anchorage for the intended fall protection load and calculate fall clearance before tying off.

Installation Overview

Follow the manufacturer's manual. The sequence below is an overview only.

  1. Inspect. Check the anchor, swivel action, puck, threads, and hardware before installation.
  2. Qualify the weld. Have an engineer or qualified person approve the weld procedure for the structure.
  3. Position. Locate the puck at the engineered anchor location on clean, prepared steel.
  4. Weld. Apply the 3/8 inch minimum fillet weld per the installation drawing, then let it cool and inspect the weld.
  5. Mount the anchor. Thread the Grade 8 bolt with lock washer through the swivel anchor into the puck and torque to 75 to 90 ft-lbs.
  6. Connect. Attach the connecting device to the D-ring and verify fall clearance before work begins.

Inspection and Retirement

Inspect before each use, with a documented competent-person inspection at least every 6 months. Pull the equipment from service for any of the conditions below.

  • D-ring and bridge: bending, cracks, nicks, or wear at the connection point.
  • Swivel action: binding, grinding, or play in the rotation or pivot; key lock fully seated.
  • Weld: cracks, undercut, corrosion, or any visible weld defect.
  • Threads and bolt: damage, corrosion, or loss of torque on the Grade 8 hardware.
  • Markings: missing or illegible product label.

Retire the anchor and remove the puck location from service immediately after any fall arrest event, any deformation or weld damage, or any condition where the system cannot be verified as safe for service.

What is the difference between fall arrest and fall restraint?
Fall arrest catches a worker after a fall happens; fall restraint prevents the worker from reaching the fall hazard in the first place.
